	A note about hormones and bone Hormone therapy does a great deal — it redistributes facial fat, softens skin texture, and changes how the face reads over time. What it cannot do is alter bone that has already matured. No length of HRT will reduce a prominent brow ridge, narrow a square jaw angle or shorten a long forehead. If those are your concerns, surgery is the only route, and waiting longer on hormones in the hope that they will change won’t help. Knowing that early saves people years.

The Procedures, Area by Area

Upper third – forehead, brow and hairline

Forehead contouring (frontal sinus setback). The definitive procedure for brow bossing. In most people the prominence sits over an air-filled frontal sinus, so simply shaving the bone isn’t enough or safe — the front wall of the sinus is removed, reshaped and repositioned flatter, then fixed. This is genuine craniofacial surgery and is the technical heart of FFS.
Orbital rim contouring. Softening the bony ridge around the eye sockets, opening up the eye area.
Brow lift. Repositioning the brows slightly higher and with a more arched shape — typically done through the same approach.
Hairline advancement / lowering. Moving the hairline forward and reshaping an M-pattern into a rounded contour.

Non-surgical treatments are genuinely useful — as a first step for people not ready for surgery, as refinement afterwards, or where the concern is soft tissue rather than bone. They are not a substitute for skeletal change, and any clinic implying filler can address brow bossing is misleading you.

Safety, Risks and Choosing a Surgeon

FFS is major surgery and deserves to be discussed as such. General risks include bleeding, infection, adverse anaesthetic reaction and unfavourable scarring. Procedure-specific risks include prolonged or permanent numbness (common temporarily, rare permanently), asymmetry, injury to the supraorbital nerve during forehead work, injury to the nerve supplying the lower lip during jaw work, and — with frontal sinus setback — sinus-related complications if the sinus is not managed correctly.
